The Dismissal

will be terminated at a future date and requested you to continue working until that date)

to prepare for a change in employment?

6.4 - Were any other people laid off in your department prior to your dismissal? If so, what period of

notice were they given by the employer?

6.5 - Is your employer alleging that it has just cause to dismiss you? If so, please describe the

allegations in as much detail as possible:

Aggravating Factors

 

7.1 - If your employer is alleging that it has just cause to dismiss you, are the allegations

clearly unsubstantiated? Please explain:

7.2 - Were you actively recruited by the employer from a previous secure employment and, if so, did

the employer offer job security as an inducement to accepting employment?

Employment Contract

 

8.1 - When you were hired, did you ever sign a written employment contract? (If “yes”, please answer

questions 8.2, 8.3, 8.4, 8.5, 8.6, 8.7, 8.8 and 8.9; if “no”, then please proceed to answer questions 8.10 and 8.11)

8.2 - Does the written employment contract contain provisions relating to dismissal? If so, please describe:
8.3 - Is the contract for a fixed term or for an indefinite period?

8.10 - Is there any document other than a written employment contract which you know of which addresses the

terms of your employment?

8.11 - Did the employer ever make any oral promises relating to the terms of your employment?
